GUIDELINES FOR MEDICATION ADMINISTRATION
** No student should carry any medication to the classroom.
** No medication including prescriptions or over the counter medications, will be administered to students without the proper authorization forms completed
If it is necessary that medication be taken during the school hours, the following is required:
1. A completed Medication Authorization, Asthma Action Plan and/or Allergy Emergency Treatment form(s) by the parent and prescribing primary care provider.
2. Mdication is to be brought to school in the original prescription bottle, labeled with the student’s name, name of medication, dosage, prescription number, and primary care provider’s name by the parent/guardian3. Non-prescription medications (such as Advil, Tylenol, or any over-the-counter medications) may not be administered without the Medication Authorization form being completed by the parent/guardian and primary care provider; these medications must come to school in their original packaging or container. No parent phone instructions can be accepted
